ABOUT THE ROLE We're Oscar, a health insurance company built around a full stack technology platform and a relentless focus on serving our members. We're hiring a Case Management Social Worker to join our Clinical Concierge Team. As a Case Management Social Worker, you will play a crucial role in educating members on improving health outcomes, assessing Social Determinants of Health (SDOH) and community resource needs, and working with other Oscar teams to ensure exceptional care for our members. WHAT YOU'LL DO As a Case Management Social Worker, you will actively assist in the coordination of care across various settings, maintain strict confidentiality and professionalism, and engage with members and providers to develop unique care plans. You will also assess and support SDOH needs of our members, educate them on resources for community support programs, and participate in crisis intervention. Additionally, you will serve as a member advocate, assist with problem-solving, and communicate with members in person, via phone, and/or secure messaging to provide education on health conditions, new medications, and procedures. WHAT YOU'LL NEED To be successful in this role, you will need 2+ years of Medical social work experience in a healthcare facility setting, 1+ years of experience in Care Coordination and Navigation, and a Master's in Social Work (MSW) from a graduate of a Nationally Accredited School approved by the Council on Social Work Education. Bonus points will be given for Licensed Master Social Worker or Licensed Clinical Social Worker, Behavioral Health Experience, Case Management experience, Complex Case Management certification, and Bilingual skills (Spanish). WHY REMOTE This is a remote position, open to candidates who reside in Arizona, Florida, Texas, and other locations. While your daily work will be completed from your home office, occasional travel may be required for team meetings and company events.
